School Form 10 (Form 137) is not subject for any modification

Director for Planning Service Roger B. Masapol of the Department of Education posted on Facebook regarding the modifications of School Form 10 (SF10) or Form 137. He addressed such modifications like adding a picture of the learner, inserting the logo of the school, putting the school logo as a big watermark are clear violations of DepEd Order No. 58, s. 2017

DepEd Order 58, s. 2017 or the Adoption of New School Forms for Kindergarten, Senior High School, Alternative Learning System, Health and Nutrition and Standardization of Permanent Records.

Masapol added that they will issue a memo to the respective officials to correct this malpractice.

“School Form 10 (Form 137) is not subject for any modification. Any proposed modifications must be submitted to CO for evaluation. We have seen several public schools who made substantial modification which chronically depleted the intent of SF10. This is a clear blatant disregard for DepEd Order 58, s. 2017. Modifications like, adding picture of the learner, inserting the logo of the school, putting the school logo as big water mark, etc are clear violation of DepEd Order. Well issue memo to the respective officials to correct this malpractice.”
