The CS Form No. 212 or Revised Personal Data Sheet is a government form to be accomplished by all government employees, and for purposes of application to a vacant position in the government.
CSC org the Civil Service Commission instructs all government employees to use only the official and revised copy of Form 212 in updating their data. There have been various copies (most are old) of the said form across the internet which are not up to date, hence considered invalid.
